What is CORE?

The CORE program serves clients who are 21 and older with Managed Medicaid (Fidelis, United Healthcare, and CDPHP) enrolled in a HARP (Health and Recovery) Plan.  Staff assist clients in reaching their person-centered goals and work with them to remain independent.

CORE removes barriers by bringing Mental Health and Substance Use Rehabilitation Services directly to individuals in non-traditional ways in their home and community settings. CORE services focus on actively creating behavior change with interventions that involve clients using and practicing and rebuilding skills, working one-on-one with providers taking action steps toward goals:

Availability to meet for
longer durations
with the clients.

Meet the client's
every week.

Flexibility with the clients to engage in more creative interventions.

Remove transportation barriers for clients; Staff will meet client's where they are at home or in the community.

CORE has the flexibility to assist individuals who have
struggled engaging in services in the past.

Hands on assistance and practice in the community to
overcome social anxiety/phobias and barriers to accessing community resources.

Who is eligible for CORE Services?

Adults 21 and older with Managed Medicaid (Fidelis, United Healthcare, and CDPHP)
and
enrolled in a HARP (Health and Recovery Plan).

Getting started.

CORE Services has a “No Wrong Door Pathway” referral process. This means that anyone can refer to the program including mental health professionals, doctors, nurses, care managers, family, and friends. Self-referrals are accepted as well! If individuals qualify for services and wish to apply, they must complete this referral form and email it to our team at core@bhsn.org.  

If you or someone you know has Managed Medicaid and you are not sure about the HARP plan, you can send an email to core@bhsn.org and we can provide you with the information.  

 

Making a referral

Referral Form has 2 sides.  One for Licensed LPHA Professionals and the other side for all other referrals as we have a Licensed LPHA on Staff to sign off.  The referral form can be filled out online and emailed core@bhsn.org.
