Job Description

Assists the Twin Oaks Supervisor with the daily operation of and quality of care provided at the community residence.  All shifts are stay awake shifts.

Essential Duties & Responsibilities: 

  • Assists residents with daily needs including but not limited to cleaning, cooking, transportation, linkage to other services
  • Oversees accuracy of documentation of client health record including but not limited to services provided, coordination of care, and medical information
  • Ensures the integrity of the medical management program is maintained and residents needs are met
  • Observes the self-administration of medications according to established protocol
  • Schedules medical appointments and coordinates medical transportation for residents both internally and through medical transportation
  • Collaborates, documents and timely communicates client incidents/events according to established protocol
  • Assists supervisor with financial management of residents funding
  • Ensures a safe and clean living environment
  • Assists in planning and coordinating social activities in conjunction with the team
  • Contributes to continuous quality improvement
  • Oversees Residential Aides in the absence of the Twin Oaks Supervisor
  • Provides resident transportation as needed
  • Participates in staff and department meetings as scheduled and attends trainings as required
  • Performs other duties consistent with position description as assigned

Requirements

  • High School diploma or equivalent and experience in human services required
  • Experience working with special needs populations required; experience working with individuals recovering from severe mental illness and/or substance use disorders preferred
  • Strong interpersonal, communication, time management, problem solving, leadership, and organizational skills required
  • CPR certified or able to achieve certification within 6 months of employment
  • Must be able to lift up to 20 lbs and have the ability to climb and descend stairs
  • Ability to work a flexible schedule to accommodate the needs of the program
  • Proficient in Microsoft Office software and in maintenance of electronic records/charts
  • Valid NYS driver’s license and reliable transportation for required local travel
  • Ability to demonstrate sound judgement and to practice within ethical parameters
