CARE MANAGEMENT SERVICES
Our care management services offer a diverse and coordinated approach to improving health outcomes and quality of life. At Behavioral Health Services North, Inc, our approach is based on the belief that the needs of our clients are best met by creating an environment that helps them organize, make sense of, and navigate today’s complex health care system.
Our proactive, integrated model blends assessments with targeted management into a comprehensive program utilized by the care management team that includes: Assessments and Care Plans; Use of comprehensive and care specific assessments to assess medical, behavioral, social and functional needs; Evidence-driven goals and care plans; Reassessments and course corrections as determined by the client’s achievements and further needs; Home visits for those seriously at risk for poor health outcomes; Established and coordinated care plans for members being discharged from a hospital, nursing home or other facility.
CHILDREN'S CARE MANAGEMENT
BHSN’s Care Management Program consists of Health Home Care Management for Medicaid clients, as well as Non-Medicaid Care Coordination services. We work with youth between the ages of 5 and 21, who have a qualifying mental health diagnosis or other chronic condition that affects their daily functioning. Youth that are referred to our program typically struggle with things such as aggression, depression, anxiety, conduct issues, self-harming, etc.
ADULT CARE MANAGEMENT
Care Management facilitates access to and coordination of the full array of primary and acute physical and behavioral health services. Services are designed to help people manage chronic health conditions, improve health outcomes and reduce the use of avoidable inpatient and emergency room care. In order to qualify for services, an individual must be on the Health Home roster or referred for services and reviewed and accepted by the Single Point of Access Committee.
