BREAKTHROUGH II HOUSING (CONGREGATE CARE LEVEL II)
Behavioral Health Services North, Inc. offers a Community Residence that provides 24/7 on-site support for persons recovering from mental illnesses that are in need of a structured living environment. The community residence is equipped with two “practice apartments” that assist individuals before transitioning into a lesser level of care.
The Apartment Program offers a less intensive level of care for persons recovering from mental illness as participants reside in either 1 or 2 bedroom apartments located within the City of Plattsburgh. All residents have their own bedrooms, however, those living in 2 bedroom units share common space such as kitchen, living room and bathroom. Case Managers meet with residents throughout the week to offer rehabilitative support services.
TWIN OAKS COMMUNITY RESIDENCE
Twin Oaks is a 20 bed community residential program providing 24/7 on-site support for men recovering from alcohol and/or chemical dependency. Participation is usually 6 months and focuses on one’s recovery and employment. Alcohol/Chemical dependency is a disease for which there is no “cure”, but there are effective treatments. Participation in a structured environment like Twin Oaks is part of the process of recovery, encouraging the development of skills to recognize and manage symptoms, and re-enter the community in a positive and constructive manner.
HELPING HANDS HOUSING (SUPPORTIVE SCATTERED SITE HOUSING)
Helping Hands Housing assists persons recovering from mental illness and their families secure and maintain safe and affordable permanent housing of their choosing within Clinton County. Recipients need to have the basic skills necessary for independent living, including the ability to self-administer and manage medications. Support services are provided as staff meet with individuals at least once a month and provide quarterly home visits. Recipients also may receive rental subsidies, assistance with furnishing dwellings, security deposits, as needed. Staff is available to discuss housing issues and advise on landlord/tenant disputes. The goal is to assist the recipient maintain affordable, safe, quality permanent housing.
HOMESTEADS ON AMPERSAND (SPECIAL NEEDS UNIT)
This safe, affordable rental housing complex conveniently located behind Consumer Square in Plattsburgh, includes 25 apartments dedicated to individuals recovering from mental illness. Financial services such as rental subsidies, assistance in furnishing apartment, and security deposits may be available depending on need. Support services are provided as staff meet with individuals at least once a month and provide quarterly home visits. The goal is to assist the recipient maintain affordable, safe, permanent housing.
“SHANNON’S HOUSE” (AKA HOUSING NORTH)
Behavioral Health Services North, Inc. owns and operates “Shannon’s House” which is comprised of four apartments in a complex within the City of Plattsburgh. This program offers single, homeless individuals recovering from mental illness, safe, affordable, permanent housing. Recipients may be eligible to receive Helping Hands Housing financial and support services while participating in the program.
NORTHWOODS APARTMENTS (BHSN UNITS)
This safe, affordable rental housing complex conveniently located behind Valcour Imaging in Plattsburgh, includes 40 apartments dedicated to homeless households that are recovering from mental illness, substance use disorders, are between the ages of 18-25, or are chronically homeless. Financial services such as rental subsidies, assistance in furnishing apartment, and security deposits may be available depending on need. Support services are available 24/7. The goal is to assist the recipient maintain affordable, safe, quality permanent housing.
APPLICATION PROCESS
The Clinton County Adult Single Point of Access (SPOA) committee will review applications for Breakthrough II Community Residences, Apartment Program, Helping Hands Housing and Homesteads on Ampersand and assess the level of care needed.
The Clinton County Housing Coalition’s Housing and Rehabilitation Selection (HARS) Committee review applications in a timely manner, ensure eligibility criteria and placement on waiting list reflect HUD S+C guidelines. Approved applicants are housed based on the order of the waiting list and priority is given to those that are considered chronically homeless.
