Breakthrough II Community Residence Program

BREAKTHROUGH II Housing

The community residence program provides a place for persons recovering from mental illnesses in need of a structured living environment. The program offers 24-hour staffed residential settings including two levels of care and services. The Supervised Residences provide an intensive level of care and the Supportive Apartments provide a less intensive level of care. An admissions/discharge committee reviews applications and assesses the level of care needed. The staff works closely with clients at all levels to coordinate participation in day treatment and other community programs to provide a full continuum of care for persons suffering from mental illness. This makes the goal of independent living a real possibility for persons making the difficult transition back to the community.

Twin Oaks community residential program for men

Twin Oaks

Twin Oaks is a 20-bed community residential program for men in chemical dependency recovery. This program lasts 6 months and is focused on recovery and employment. Chemical dependency is a disease for which there is no "cure," but there are effective treatments. Participation in a structured environment like Twin Oaks is part of the process of recovery, encouraging the development of skills to recognize and manage symptoms, and reenter the community in a positive and constructive manner.

supported housing Programs:

HELPING HANDS HOUSING
Helping Hands Housing provides a stable living environment to persons recovering from mental illness. Recipients need to have the basic skills necessary for independent living, including the ability to self-medicate. Services are at a minimum in this program, however recipients receive rent subsidies, start-up costs such as furnishing expenses, security deposits, utility subsidies, monthly contacts, and quarterly home visits. Staff is available to discuss housing issues and advise on landlord/tenant disputes. The goal is to assist the recipient maintain affordable, safe, quality housing after being a part of Helping Hands Housing.

HOUSING NORTH
BHSN owns and operates Housing North which is comprised of four individual apartments in a complex within the City of Plattsburgh where support services are available for tenants. With the assistance of the Continuum of Care Committee comprised of a variety of or representatives from community support services agencies relating to safe, affordable housing for low-income individuals.

SHELTER NORTH
Established in 2005, Shelter North leases several units in the City of Plattsburgh as part of the Shelter Plus Care Program funded by a grant from HUD. Shelter North is designed to provide affordable housing and supportive services to homeless adults who have mental illness and substance abuse problems. It is jointly administered by BHSN and the Clinton County Department of Social Services.

HOMESTEADS ON AMPERSAND
Opening in summer 2015, this affordable rental housing in Plattsburgh includes a number dedicated to individuals recovering from mental illness in a safe, afforable, quality permanent housing situation. Support services, as needed, are provided to help each individual maintain housing and well-being.
